HISTORY AND PHYSICAL HPI: 79M, independent at baseline, with 2 days dysuria/frequency and 1 day acute confusion and somnolence. UA consistent with UTI. CT head negative. Ammonia, TSH, glucose not explanatory. PMH: HTN, T2DM (A1c 6.9%), HLD, CAD s/p PCI to LAD 2019, no angina since. Meds: metformin 1000 mg BID, lisinopril 10 mg, atorvastatin 80 mg, aspirin 81 mg. Exam: T 37.9, HR 88, BP 142/80. Somnolent, oriented to self only, inattentive (cannot say days of week backward). No asterixis. Nonfocal neuro exam. ASSESSMENT/PLAN: 1. Acute metabolic encephalopathy due to UTI - acute global change in mental status from fully independent baseline, nonfocal exam, negative CT head, alternative toxic-metabolic causes excluded. Treat infection, delirium precautions, avoid sedatives/anticholinergics, sleep hygiene, hold home meds that could contribute (none). 2. UTI (acute cystitis vs early pyelo) - ceftriaxone 1 g daily, f/u urine cx. No evidence of sepsis: hemodynamically stable, lactate 1.3, no other organ dysfunction. 3. T2DM - hold metformin; SSI. 4. HTN - continue lisinopril. 5. CAD s/p PCI - asymptomatic; continue aspirin, statin. 6. HLD - atorvastatin 80 mg. DVT ppx enoxaparin. Full code.
